Student medical/health forms for the school year 2010/2011 have been sent to parents/guardians with tuition invoices via email. The forms can also be obtained from this page or the admission page of this website.


Important Note:  The following forms are for new and returning ACST students.  The medical/health forms must be submitted in a sealed envelope to the nurse's office between Aug 18 and Aug. 23, 2010.

Hand sanitizers and tissues have been distributed to all rooms/areas at A.C.S.T.  There are several brands of hand sanitizers available on the local market. When purchasing a hand sanitizer, the alcohol content must be at least 60% to be effective.

Good hand washing techniques must be encouraged to everyone. Hands should be washed/sanitized:

-Before and after eating.
-Before preparing food.
-After going to the bathroom.
-After changing diapers or cleaning up a child who has gone to the bathroom.
-Before and after tending to someone who is sick.
-After handling an animal or animal waste.
-After handling garbage.
-Before and after treating a cut or wound.

Four steps for proper use of hand sanitizers:
-Make sure all organic matter is removed from hands.
-All visible organic matter (for example:  dirt)  must be removed from hands prior to applying waterless hand sanitizer.  Hand sanitizer must contain at least 60% alcohol to be effective.
-Apply a dime size amount of hand sanitizer to the palm of one hand.
-Rub hands together covering all hands and fingers.
-Rub until hand sanitizer is absorbed.

*PLEASE MONITOR CHILDREN WHEN THEY ARE USING HAND SANITIZERS.  If you observe a child trying to sniff or lick hand sanitizer, please correct them. Hand sanitizers are being used to prevent the spread of germs. Hand sanitizers should not be abused by anyone.*
